Template for 8 1/2 X 11 notecard holder

I'm looking for a template to make a notecard holder to hold 4 to 6
cards and envelopes out of one sheet of cardstock. I can get cardstock
cheaper than I can get the cheap acrylic boxes online.:laugh:

Can anyone help?
